How do I pay my plan premium?
You have three options to pay your plan premium. You can indicate which option you choose on the enrollment form or short enrollment form:
Electronic funds transfer (EFT). Your bank can automatically transfer your premium to us.
Monthly bill. We can send you a bill and you can send us a check. If you choose to pay by check please remember to include it with your enrollment or short enrollment form.
Have it withheld from your Social Security benefit check. (Note: If you decide to have your plan premium withheld it from your Social Security benefit check or move from withhold to direct bill, it could take up to three months for it to take effect and you will be ultimately be held responsible for those premiums.)
If you want to sign up for Electronic Funds Transfer (EFT) to automatically withdraw your premium from your bank account, please complete this form and send it to us:
