Now Health First Health Plans members can have a personal Health Coach, at no cost — by phone, 24 hours a day, 7 days a week — through our program. If you have questions about health and wellness, or if you have questions about a screening test or procedure, your Health Coach can help you understand your options.
A Health Coach is a trained, health care professional who can provide you with one-on-one information on a variety of health issues, help you monitor your health needs, and help you work more closely with your doctor.
With the Health Dialog Program, you can:
- Talk with a Health Coach anytime, day or night. Just call 1-800-308-5848 to speak with a knowledgeable and caring health care professional such as a nurse, dietitian, or respiratory therapist.
- Receive personalized check-in calls from a Health Coach. You can talk to a Health Coach about your upcoming appointments, medications, diet, or exercise plan.
- Get additional health information mailed to your home at no cost.
- Learn about new techniques to help you take control of your health or specific conditions.
- Use online services featuring 24/7 access to health tools, decision support tools, and the Healthwise (R) Knowledgebase — an online encyclopedia with thousands of easy-to-understand articles on various health topics as well as messaging with a Health Coach.
