If you'd like to become appointed with Health First Insurance, we require the following items:

•  Health First Agent Agreement
•  Health First Business Associate Agreement
•  IRS W-9 form (Request for Taxpayer Identification Number and Certification)
• Proof of License—All soliciting agents are required to hold an active health insurance license (2-40 or 2-15) with the state of Florida or a Florida non-resident license. Please provide a copy of license during your initial contracting period. Verification of your license will automatically be conducted upon renewal of your appointment.
• Proof of Errors and Omissions (E&O) coverage—Each agent is required to carry and maintain E&O Coverage. Please include proof of coverage with your initial appointment request and any renewal request. Failure to provide such proof will terminate your HFI appointment.
• A copy of your AHIP Certification (or similar)—All agents are required to obtain America’s Health Insurance Plan (AHIP) certification with a passing score of 90% or above in Marketing Medicare Advantage and Part D Prescription Drug Plans—Understanding Medicare Basics, Plan Types, and Marketing and Enrollment Requirements (or a similar CMS-approved Medicare certification). Please include a copy of your current certification and transcript showing your score with your initial appointment request. You must also renew your certification and send us a copy each year for the duration of your appointment. Failure to provide this each year will terminate your HFI appointment.
• A check for $63 payable to Health First Insurance, Inc. for the appointment fee.
