Cardiac Transition Program: Taking the next step

Upon the recommendations of your physician, you may be encouraged as part of your recovery to join the Cardiac Transition program at one of our Pro-Health & Fitness Centers and take personal charge of your continuing cardiac rehabilitation. Our Pro-Health & Fitness staff provides a comprehensive orientation program that will introduce you to the center's state-of-the-art exercise equipment designed to meet your fitness goals.

Cardiac Transition program helps you

  • Feel better faster
  • Continue getting stronger
  • Reduce risks of future heart problems
  • Reduce stress
  • Live healthier

Building your support network

Breaking old habits and learning new ones can be stressful, but family members and friends can make a dramatic difference in your recovery process. They can give you the emotional support you need to help you adjust to a new, healthier lifestyle. We encourage family involvement and invite interested family members to participate in the patient's education sessions.

You and your family may also benefit from the support of others who have heart disease. There are many local support groups that may be of help. Web links to many of these resources are available on this page.

Thank you for choosing the Cardiac Services at Holmes Regional Medical Center. Your heart is in the right place at Health First.
