How to Get Started

If a patient qualifies for admission to Hospice Care, all you need to do is call Hospice of Health First at
321-952-0494. We will take it from there and make the transition as smooth as possible.

Referral Information
Referrals to our Hospice program come from many sources, including patient and family requests. However, the patient's attending physician must authorize services and all care is coordinated and progress reported to the patient's primary attending physician. If you or a loved one is in need of Hospice Care, you can ask your doctor for a referral to Hospice of Health First.

• A member of the National Hospice and Palliative Care Organization and Florida Hospices and Palliative Care
