Policies

New patients should arrive at least 15 minutes before their scheduled appointments to complete all paperwork. They should bring their insurance cards, a government-issued photo ID, and lists of their medicines and prior operations. Co-pays and any outstanding balances are due at the time of each visit. Generally, operations or procedures will not take place on the day of a first visit.
