Health First Foundation Scholarship

2013 Scholarship Information

The Health First Foundation Scholarship Program is made possible by charitable fund raising activities and through generous donations from individual benefactors. Scholarships are awarded exclusively to students pursuing degrees and studies in the medical field. Recipients are selected on the basis of financial need, character, and academic achievement. Each scholarship presented is in the amount of $1,000 per school year.

First year recipients who continue their studies and maintain a GPA of 3.0 or higher will automatically qualify for another $1,000 scholarship awarded in their second year of study. All scholarship payments will be sent directly to the student’s college or university upon confirmation of enrollment and a class schedule is provided. Applicants may be asked to meet with the Scholarship Committee for a personal interview after all application materials have been reviewed. A letter of determination will be sent to all applicants within 3 to 4 weeks of the application deadline.

Guidelines and Requirements (You must meet all of those listed):

	Must be resident of Brevard County.
	Graduating senior from a Brevard County High School.
	Pursuing a career in a medical or healthcare-related field.
	GPA of a 3.0 or above.

Required Application and Supporting Documents

Your scholarship application MUST be submitted and supporting documents be postmarked no later than March 22, 2013

	General Application Form	By submitting one online application, you will be considered for all scholarships available.
	Essay	On a separate sheet of paper, write a brief essay, 500 words maximum-doubled spaced, explaining your need for a scholarship. Include your achievements to date, and your personal and professional goals.
	Request for Student Records	You must complete the top portion of this form. The remainder of the form is to be completed by your school's Principal or Guidance Counselor. They should attach your transcript with the Request for Student Records Form, and mail directly to the address below.
	Two Letters of Reference or Confidential Reference Request Form	Your application must include letters from two individuals. Suggested persons are teachers, employers, clergy, or professionals in a medical field. These persons should not be related to the applicant.  Must be mailed directly to the address below.
