The Medicare Program rates all health and prescription drug plans each year, based on a plan's quality and performace. Medicare Plan Ratings help you to know how good a job our plan is doing. You can use this Plan Rating to compare our plan's performace to other plans. View our Stars Ratings. Medicare evaluates plans based on a 5-Star rating system. Star Ratings are calculated each year and may change from one year to the next.

In-Home Health Assessments for Medicare Members
A Nurse Practitioner from our partner, Matrix Medical Network, can spend time with you to answer any and all questions about your health, medications or chronic conditions. In addition, the Nurse Practitioner will perform simple exams like checking your blood pressure and heart rate and the information you share will be given to your doctor to help better manager your health.
