Health First Physicians is proud to announce that Dr. David Norris, an Internal Medicine Specialist, has been awarded "The National Committee for Quality Assurance Physician Practice Connections-Patient-Centered Medical Home recognition."
It is a long name for successfully treating patients as part of a long-term plan of care, rather than simply treating patients in a series of episodic office visits. In other words, Dr. Norris uses a team approach to better coordinate various treatments across the healthcare system, keeping his patients healthier.
At a time when healthcare reform focuses on waste, duplication, and unneeded testing in the system, this team approach can reduce costs, improve care, and lead to healthier patients.
The NCQA Patient-Centered Medical Home model is the most widely accepted such models in the country. Patient-Centered Medical Home is a way to move primary care into the 21st century.
